〔備考〕
１ 令第１条第２項の規定による命令に基づく手続の補正（法第18条第２項（同項の表３の項に掲げる部分に限る。）の規定により納付すべき手数料の納付の補正に限る。）をするときは、表題を「CORRECTION」とする。
２ その他は、様式第１の備考１から５まで、９、13、14、20及び21、様式第１の２の備考１から５まで、７及び８、様式第２の３の備考１、様式第２の４の備考２及び３、様式第11の７の備考３、様式第18の２の備考１並びに様式第27の２の備考２と同様とする。この場合において、様式第18の２の備考１中「Amount of the Additional Fee」とあるのは「Kind of Fee and Amount」と、「「Amount of the Additional Fee」の欄の次に「Credit Card Payment」」とあるのは「「Kind of Fee and Amount」の欄の次に「Credit Card Payment」」と読み替えるものとする。
