５　Subject Matter of Rectification

１　「Item to be Rectified」の欄には、「Box No. II APPLICANT of the Request」のように訂正をする書類名と訂正をする箇所を記載する。

２　「Subject Matter of Rectification」の欄には、「As per the attached sheets」と記載するとともに訂正事項を指摘し、訂正のための差替え用紙を別紙として添付する。ただし、その訂正に係る事項についての記録原本及び訂正の対象とする書類への書き換えが容易にできるときは差替え用紙によることを要しない。また、第50条の３第８項の規定により第77条第１項の規定による訂正後の明細書に記載した配列表を記録した磁気ディスクを添付して提出するときは、「Information Such As Recording Form of Magnetic Disk」（原則として、「Applicant」、「Agent」、「Identification of the International Application」、「Title of Invention」、「Character Code」、「File Name」及び「Contact Person (Tel (Fax), Name)」の項目を設けて記載することにより作成する。）を添付し、「Subject Matter of Rectification」の欄には「As per the attached Magnetic Disk」のように記載するとともに、訂正事項を指摘し、差替え用紙の添付は不要とする。

３　その他は、様式第１の備考１から５まで、９、13、14、20及び21、様式第１の２の備考１から５まで、７及び８、様式第２の３の備考１並びに様式第２の４の備考２及び３と同様とする。
