〔備考〕

１　国際調査機関又は国際予備審査機関のみに対する復代理人の選任を届け出るときは、「Sub-Agent Appointed」の次に、「Scope of power of Attorney」の欄を設けて、その旨を記載する。

２　「Sub-Agent」の欄には、その氏名の記載に併せて、その氏名の前に「Attorney at law」又は「Patent attorney」のうち該当するものを記載する。

３　復代理人によるときは代理人の署名又は印は不要とし、復代理人によらないときは「Sub-Agent」の欄を設けるには及ばない。

４　その他は、様式第１の備考１から５まで、９、13、14、20及び21、様式第１の２の備考１から５まで、７及び８並びに様式第２の３の備考１と同様とする。
