〔備考〕
１ 代理人（代表者）が辞任を届け出るときは、表題を「NOTIFICATION OF RENUNCIATION OF POWER OF ATTORNEY」とし、「Agent (Common Representative) Withdrawn」を「Agent (Common Representative) Resigned」とする。
２ 国際調査機関又は国際予備審査機関のみに対する代理人の解任又は辞任を届け出るときは、「Agent (Common Representative) Withdrawn (Resigned)」の次に「Scope of power of Attorney」の欄を設けて、その旨を記載する。
３ その他は、様式第１の備考１から５まで、９、11、13、14、20及び21並びに様式第１の２の備考１から５まで、７及び８と同様とする。
